Please use FET form for entering individual FET or Donor FET details to the database. Use this form in case you have already entered the IVF/ICSI details using the IVF-ET form some time ago and presently looking for entering FET details only. By this way, you will be generating multiple case IDs for the same patient. You need to copy these IDs on the physical files for future reference.
